To apply to the Definition of Success Mentorship Program, please fill out our online form or mail a printed application to the address below. Space in the program is limited, and we want to make sure to enroll kids who will learn and benefit.

Definition of Success is not a 501c(3) certified non-profit organization ... yet.  We are in the process of applying for this certification. In the meantime, charitable, tax-deductible contributions can be made through Access to Opportunity (www.AccessToOpportunity.org), a 501c(3) organization. Contributions to Access to Opportunity earmarked for Definition of Success will be transferred in full to our program.
